Dear after-school class providers:

Thank you for your interest in participating in Bonny Slope’s after school enrichment program.  Our goal is to give our students additional opportunities for intellectual, artistic and social development with classes that appeal to a broad variety of interests and ages.  Three sessions of after-school programs will be offered during the school year.

FORM SUBMISSION

Working with schools in the Beaverton School District requires that you:

Complete a Facility Use Application  and pay facility use fees.  The application should be sent directly to the school, attention Tamara Angier, for approval. You will then receive an invoice from the district office payable upon receipt.
Submit your flyer to the district office for review.  Requirements of the process can be found at http://www.beaverton.k12.or.us/home/community/

Once your flyer has been approved, please email a PDF copy to omalleymeg@hotmail.com and drop off 30 copies at the Bonny Slope Elementary School office. Our classes are advertised through an electronic catalog. Deadlines must be adhered to. If you have to cancel a class, it is your responsibility to notify the students and the school. Email a list of enrollees to omalleymeg@hotmail.com when registration closes.

STUDENT SAFETY

School will end at 3:05 and classes should begin at 3:10. Students are in your care from 3:10 until they have been released as indicated by parent. Parents, or another designated adult, must pick up children up at your room’s door. Children in after school care are escorted by you to the school cafeteria.  Children may be released on their own if s/he will walk or bike home per parent request.

Email PDF file & drop off 60 flyer copies: 9/15/11
Registration DEADLINE: 9/29/11
Turn in list of enrollees: 9/30/11 by noon
Classes begin: Week of 10/3/11
Classes end & pass out evaluations: varied

DEADLINES FOR WINTER TERM

Submit flyer to district office: ASAP (Last date to submit for review by BSD for Winter Term: 11/4/11)
Download & Submit Facilities Use Application to Bonny Slope office: ASAP (11/14/11)
Email PDF file & drop off 30 flyer copies: 11/14/11

Open registration for classes : 11/18/11
Registration DEADLINE: 12/9/11
Turn in list of enrollees: 12/14/11

Classes begin: Week of 1/9/12
Classes end & pass out evaluations: varied

DEADLINES FOR SPRING TERM

Submit flyer to district office: ASAP (Last date to submit for review by BSD for Spring Term: 2/3/12)
Download & Submit Facilities Use Application to Bonny Slope office: ASAP (3/2/12)
Email PDF file & drop off 30 copies to the main office: 2/27/12

Open registration for classes: 3/2/12
Registration DEADLINE: 3/23/12
Turn in list of enrollees: 4/2/12

Classes begin: Week of 4/9/12

Classes end & pass out evaluations: varied
